Tdap (Whopping Cough) Shot Record Needed

Whooping cough, also known as pertussis, continues to be widespread in California. To help stop its spread, all students in 7th through 12th grade are required by law to have proof of a whooping cough booster (‘Tdap’) shot, or file an exemption, in order to attend classes this year. Students will not be allowed to attend classes on the first day of school without a shot record.
We need a booster shot record from your son/daughter. If he/she hasn’t had a booster shot yet, please schedule an appointment immediately with your regular doctor. Vaccine may also be available at Humboldt County Department of Health, Open Door Community Health and Eureka Pediatrics. If he/she has already received the booster shot, please give us a copy of the shot record ASAP to avoid your student being excluded from school.
