Incoming 7th - 12th Grade Immunization Requirement

Beginning July 1, 2011 all students entering 7th through 12th grades in the 2011-2012 school year in California must be immunized with a pertussis (whooping cough) vaccine booster called Tdap.  For more information visit the Shots for Schools website:

To help make the start of school next year a smooth one, we ask that all parents of entering 7th – 12th graders present evidence of immunization early.  Please do this as soon as possible after your son/daughter receives the Tdap booster.  Please do NOT wait until next fall.  Before the end of this school year, bring evidence of the Tdap booster to the school secretary where your student currently attends school.  After school is out - and over the summer – bring it to the District Office, attention Educational Services Dept. Make sure your student’s name is clearly printed on the form and the school he/she will be entering.

Humboldt County Public Health has made special arrangements to meet the need for Tdap immunizations for students entering 7th grade - 12th grades.

During July and August  the Public Health Clinic in Eureka will provide entering 7th - 12th graders with their Tdap immunization.

Walk-ins for parent-accompanied students, ages 11-18*
ONLY on Thursday afternoons from 1 – 4 p.m.
*18 year olds can present without their parent
